If you are mailing your quilt to me, or if you would like to drop off and pick up your quilt at Scarlet Thread Fabrics in Webster or at Patricia's Fabric House in E. Rochester, please complete the Order Form below.
If you would like to come to my studio in Palmyra, NY, just give me a call for an appointment. We can fill out the form together when you are here.
Be sure to include the dimensions of your quilt and a brief description of the quilt colors and pattern. You may choose your own thread colors from the Available Threads page and a pattern from the Available Patterns page. Or if you prefer I will choose what I feel is appropriate colors and patterns.
Please call me after you have completed the order form and we will determine the cost, the expected completion date, and at which location you plan to drop off your quilt.
Pin the order form and your personal check or money order to your quilt. Place your quilt in a plastic bag, and tape the bag closed. Tape a piece of paper with your name, address and phone number on it to the outside of the bag also.
If you are mailing it, please package it securely (and do not indicate QUILT on the outside of the package). Also, please add the return postage to the total amount of the check or money order.
If you drop off your quilt at Scarlet Thread or Patricia's, please give me a quick call to let me know it is there. I will give you a call when it has been completed and dropped off at the same location.
